Nope, you haven't lost your memory! The number should have been EOTZ-7515-B. It's Dorman Part# 14553. Your link didn't have it listed, but you can find it at this link (It takes a minute to completely load):

Good to know the Alzheimer's hasn't set in yet:D . Tallguy take note: On both of these forks the slave cylinder end is not drilled. You will want to drill a hole in the fork before you mount it (that will make adjusting the clutch a bit easier). Here's a shot of mine:
