What clutch slave is this?

I need to replace this clutch slave. Can anyone tell me what brand/model this is and where I can get a replacement. Thanks!

Go to any good parts shop, (I got mine from Carquest).
Ask for a EIS EW33721; it's a 1 in dia slave; think I paid about $40 gn |
