Mcleod 1400 Blown After 250 miles

Hey guys I have a superformance cobra 2004 SPF# 1983, With a Kieth Craft 496. I was trying to fix a few oil leaks one leak was comming from the internal throw out bearing. We decided to replace it an so we did. We replaced it with a McLeod 1400 Internal. I put about 250 Miles on it and it blew. When we pulled it out to take a look it was missing two screws an 4 others were not tight at all . I posted a link to a pic of it. The two missing screws I have circled in yellow. The one on the right you could see an o-ring comming out of it. If been reading some threads that state some leak issues with this slave cylinder but nothing like this . Any thoughts ? Please advise
