Usually when a clutch is starting to go, it will start slipping in the higher gears like that. Will it slip if you leave it in 5th gear, load it at a low rpm, then get into the throttle?

If it's a regular organic/organic disc with a diaphragm plate, chances are that you probably have more engine than what the clutch can handle. Combinations like that are generally good for 300-350hp.
