Default Question on adjust of front toe in alignment?

The manual says "Loosen the jam nut on each tie rod. Rotate the inner tie rod inside the rack boot to adjust." Seemed pretty elementary and the sleeve nut on the end of the rack assembly has flats for an open end wrench. But, when I turn it to thread forward or backward on the threaded tie rod end, the boot twists with it. I don't want to twist it around and damage the boot. I tried holding the end of the boot to see if the sleeve nut would break free and rotate inside it but it wouldn't. The boot is tight on the end of the sleeve nut. I guess cutting the zip tie strap securing the end is an option and then replace it later.

The term "inside the rack boot" isn't making sense to me unless it will turn without twisting the boot? What am I missing here - I haven't messed with many rack and pinion assemblies?
