I'm afraid I'm a little light on the originality part of this but I just went through re-doing mine. I have a Lakewood scatter shield and the original slave cylinder is a Girling. Top shelf racing stuff from the day. The mounting brackets on every set up I've seen are home made. TRW apparently bought out Girling and they have essentially killed the company. I did find a supply house in the UK that still stocks a large number of OEM Girling parts. They are PowerTrack LTD and have a web site. My slave cylinder has the following numbers associated with it - BS6 - 64677346. This is not a part number but PowerTrack cross referenced it to the correct cylinder. I bought a rebuild kit from them and they were pretty good to deal with. A new cylinder is about 56 Pounds or $100.
