Why not just remove the two bolts that fasten the lever to the shift mechanism? Then slide the lever through the boot, reapply some blue Loctite and refasten to proper torque? Then slide the trim ring over the whole assembly, fasten with the four Philips head screws and you're done. No need to stretch a rubber boot that was never designed to be stretched that way.
