It was quite a while ago and I don't remember the details. It was some sort of ground fault. Even now, it doesn't quite make sense to me that an electrical fault would cause a pinhole in the tubing, but it did. It was only after very careful examination after it failed the second time that I took a very close look at the hose and saw a tiny electrical burn mark. Only then was I sure I had an electrical fault. I fixed the fault and I've had no issues since.
