Ck the harmonic balancer--sounds like it has rotated out of spec, which means that the outer ring has shifted in relation to the inner one...with the motor off, see if you can make the two rings with some white paint ( a small line will do) and run the motor for a minute or two, than check and see if they have shifted. If not, see if you can get a hand on the outer ring and turn it by hand with a little effort...I doubt VERY seriously that a machine shops balancer was out of spec enough to cause this much imbalance!
