Head gasket or cracked (head/block). If you had
everyrthing "magged" prior to assembly and it checked out then it is probably a gasket. Put a bead of silicone around the water passage at the front of the manifold prior to assembly. If you can, prior to putting the manifold back on do a leakdown check on that cylinder (#5) to check the integrety of the head gasket.

The water passage on the intake manifold to the head can sometimes be difficult to seal. Be sure you have the correct gaskets.
