GFUJI is correct. It is a 1967 Shelby GT500 dual quad manifold.
It is also considered a medium riser. I've been running this intake on my 427SC replica for the past three years. It is an extremely good performer. Great bottom end and it pulls hard through 6000+ RPM. It is a real impressive and fine looking piece. When you undo the latches and raise the bonnet you get the "Wow" factor. They are a bit pricey but if you can locate one grab it.
