Procedure "A" - Remove the shifter lever from the mechanism (two bolts) and pass the lower end through the top of the boot first. Then re-attach the lever to the mechanism before bolting the boot back to the trans tunnel.

Procedure "B" - A pair of channel locks and a twist of the wrist should do the trick.
