About CAT specifically, I don't know.
From what I was told by a couple engine builders, they come from China which is where a lot of cast/forged stuff is coming from now. Crankshafts are being cast/forged in china and rough finished, then sent to the U.S. for final grinding. I've been told the same is true for the rods.
