Step one is to figure out the spacing and angle of the pedals. I just found the most comfortable heel/toe car I have driven and measured that (a BMW). Then, I made the pedal box that would fit the chassis and let me position the pedals in exactly in the right place. In my case the pedal box was off to the right of the perfect pedal locations. So, I had to heat the pedal arms with a torch and bend them about an inch to the left.
That's the best place to start.
