"YR 1st SOLD" on your registration should be 0000, so that's correct.
The year of the car (*YR on the registration form) will probably be the first year the car was registered in AZ. My registration indicates that my cars is a 2007 SPCNS.
At this point, you only need to visit the CHP. The visit to the BAR is done after you've been issued your SB100 sequence number. The DMV may also require that you get a brake and safety check. The DMV isn't consistent regarding the safety check, so check the top of your paperwork.
Best of luck with the SB100. The process is fairly straightforward (once you've submitted the correct forms and paid your fees at the DMV) and is really nothing that you want/need to outsource to someone else.
