Both systems could use a strict ID system with medical records automated and accessible through the ID - illegals would have to go away. Also, a co-pay after, say 3 doctor visits a year seems like a good thing to consider.

You got that right. I can't understand why we (US & Canada) allow illegals to have priviledges to which they have not, and will not, contribute.
Unfortunately, the law up here is for Universal coverage and the way it was written, there cannot be any user fees. This was fine when this was introduced about 45 years ago but I think that this must now change.
Our Medicare card numbers are not tied to our social insurance numbers which is plain stupid. I guess that may infringe on someone's right to defraud the system.
