I don't know if there are any companies out there that have what you need on the shelf. Sounds like a custom order.

There is a guy on Long Island in NY who does some really nice custom work. The guy's name is Red and his shop is called Headertech - Tel. No. (631) 242-8154

Give him a call and explain what you need and I'm sure he can make something to fit the bill.
