Health care is rationed to some degree in every country, some more so in others.
In Mexico, you're right, everybody gets the same, equal care, that is equal substandard healthcare. You're right in that it is equal.
I've never lived in Mexico, but my secretary's sister did for several years. Her sister and brother-in-law were transferred there, white collar jobs. Lived in a gated community, feared for their and their children's life every day/night, slept in the same bedroom for fear of kidnapping. Stated the schools, healthcare, everything was crap. Couldn't wait to get back to the US.
From what you're saying as an expert, me, not an expert in Mexican society, guess they must have been in a very isolated part of Mexico.
