Gregg emailed the hi-res pics to our HCC mailing list. After looking closely at them, it looks pretty obvious that the pin nuts came lose and let go of the studs. The one that sheared was the last soldier hanging on. I suspect when Gregg separates the wheel from the adapter he'll find that the 4 pin nuts came lose and then #5 snapped under weight/stress. Also, his studs look like they have been cut off, and don't look nearly long enough to pass through the adapter and securely into the pin nuts.

Default Recurring maintenance item?

Is torquing down the pin nuts considered to be a recurring maintenance item?

Hell I though torquing down EVERYTHING on these cars is a recurring mtc item.

Seriously I don't know, but I sure sleep better if I pull the wheels and take a peek around every 5-600 miles. But the new Kirkham nuts are excellent and mine have not moved in 2k miles. I used a small amount of blue Loctite just to help them behave.

You know, the cost difference on the ERA cars to upgrade to the six pin, one-piece hubs over that of the Trigo or Vintage five pin adapters is about a thousand bucks -- that's $250 a wheel. This thread was really one of the more "scary" break down threads; it easily could have turned out much, much worse (it's not like "my car keeps overheating"). By any chance do the newer cars offer a six pin option? You know, we've seen more than a couple of threads in the last few years dealing with 5-pins cracking and after reading this thread a thousand bucks doesn't seem like much at all.
