For flats I carry:
6lb lead hammer
ANTI-SEIZE!! (noone mentioned it in their kits).
Safety wire and pliers
No room for the rest of the stuff like jacks. So I carry a cell phone and AAA card. Essentially I carry what I expect the AAA guy won't have to remove the tire. The fix-a-flat kit might be useful and not take up too much space.
I also carry a spare engine belt and on longer trips add the sockets etc.
