... and you should have a rev limiter. If your floor mats jump up on the pedal, or you bust your throttle springs, or if the linkage just sticks at WOT, you can throw your clutch in and the the engine will be bouncing off the rev limiter until you regain your senses and turn the key off.

As Ernie said, Flip Flops are as bad as non fitting floor mats. Also from personal experience and a broken toe. My floor mats I took and measured and then cut them so they lay flat and the throttle can't get to them. The carpet I had molded so it was out of the way of all the pedals.
