The DMV there should have a memo that they can look at telling them just what forms and procedures to use with SB-100. I am sure they issued them to all DMVs since there was so much confusion when it first became law. If the clerk can't help you ask for a supervisor and tell them to look at the memo or instructions that were sent out for dealing with SB-100. Our DMV has did enough of them now that most of the clerks don't even have to refer to any references.
