My 69 Mach 1 had a factory flip top gas cap, no secondary cap. There was a spring loaded seal that sat against the filler neck when locking the flip top down.

Safety and liability. Ford replaced the Mustangs "flip-open" caps 15 years ago with a replacement unit with a conventional type cap hidden by the flip door. And that's for cars from 1970! They stopped supplying the original factory type caps as service replacement parts because of liability concerns.
