Doug,
What about a flat lobe on the cam for the fuel pump push rod.
Maybe a bent fuel pump push rod.
Hook up an electric fuel pump in line with the fuel pressure regulator and the fuel filter.
You want to elminate 1 thing at a time until you find the source.
