Default T5Z problem

Last year I posted that I was having a problem with my rebuilt T-5 and after much thought and good comments from CC members I decided to make a major change.
I went with a new Ford Motorsports 10.5" Clutch, new hyd slave cylinder set-up instead of cable, new pedal assembly, went to manual brakes instead of power and put in a new Tremec T-5Z from Summit Racing.
It was a lot of work on my back in the garage, but I was very satisfied with my changes. I put on about 1000 trouble free miles until this past weekend at London LCS. I traveled down to London on Saturday morning looking to enjoy the day with everyone and the cars. That trip was the most at a continuous speed, before that just cruising around, never getting into 5th gear around town.
As I pulled into London I noticed that there was a noise and severe vibration in the tranny when I put the clutch in while in gear. 1st, 2nd 3rd, didn't matter. If the car was stopped I could change gears with no noise, but if I was moving, noise and vibration.
I hung around until 2:00 and started up to go home, no noise or vibration. So I took off for Cleveland. Going down and back I was moving between 70 and 75 right around 2000 RPM in 5th gear. Everything fine untill I went to get off at the exit. There was the noise and vibration as I put the clutch in while slowing down to downshift.
Put the car up on the jack stands and ran the car like Ferris Buehler between 1st and 2nd up to 2000 RPM and changed gears with no appreciable noise or vibration.
While I had the car up I decided to change the fluid. Put in 2.5 qts of new Red Line D4 ATF Synthetic.
Took the car out for a breif ride tonight and there it was, noise and vibration.

Help, sorry for the story length, any help would be appreciated.
